Rider Waiver

This waiver is to be signed by any and all attempting to test ride GOTRAX electric scooters, electric bikes or other GOTRAX branded products. Please be sure to read thoroughly through the entire waiver and let us know if you have any questions.  

I, the undersigned participant, hereby acknowledge and agree to the terms and conditions outlined in this Liability Waiver and Release Form ("Form") in consideration of being allowed to participate in the activity or event organized by Go Labs inc DBA GOTRAX, hereinafter referred to as the "Event."

Assumption of Risk: I understand that participation in the Event involves certain inherent risks, including, but not limited to, physical injury, property damage, and loss. I voluntarily assume all such risks associated with my participation in the Event. I understand that Go Labs inc DBA GOTRAX has taken reasonable measures to minimize risks, but I acknowledge that accidents and incidents can still occur despite these efforts.

Release and Waiver: In consideration of being permitted to participate in the Event, I hereby release, discharge, and hold harmless Go Labs inc DBA GOTRAX, its directors, officers, employees, volunteers, agents, and representatives (collectively referred to as "Releasees"), from any and all liability, claims, demands, actions, or causes of action arising out of or related to any loss, injury, or damage, whether to person or property, that I may suffer or sustain while participating in the Event.

Indemnification: I agree to indemnify and hold harmless the Releasees from any claims, demands, actions, liabilities, or expenses (including attorney's fees) arising from or related to my participation in the Event or any breach of this Form by me.

Medical Treatment: I authorize Go Labs inc DBA GOTRAX and its representatives to obtain medical treatment for me in case of injury or illness during the Event, and I agree to be financially responsible for any medical treatment rendered.

Photography and Likeness Release: I grant Go Labs inc DBA GOTRAX the right to take photographs, video recordings, and/or audio recordings of me during the Event and to use my name, likeness, voice, and biographical information in connection with promotional materials and media related to the Event.

Knowing and Voluntary Execution: I acknowledge that I have carefully read and fully understand the terms and conditions of this Form. I am aware that I am releasing certain legal rights I may have otherwise and voluntarily enter into this Form without any coercion or undue influence.

Governing Law and Severability: This Form shall be governed by and construed in accordance with the laws of Florida. If any provision of this Form is deemed invalid or unenforceable, the remaining provisions shall remain in full force and effect.

By signing below, I acknowledge that I have read, understood, and voluntarily agreed to the terms and conditions outlined in this Liability Waiver and Release Form.
